Síndrome de la silla turca vacía o aracnoidocele intraselar: una revisión de la literatura

La silla turca vacía es una entidad neuro radiológica que se caracteriza por la herniación del espacio subaracnoideo dentro de la silla turca, ocasionando una compresión de la hipófisis contra el suelo selar. Su prevalencia es desconocida, sin embargo, las estimaciones oscilan entre el 2% y el 20%, la cual puede ser aún mayor debido al uso generalizado de la tomografía computarizada y la resonancia magnética craneal, lo que ha hecho del diagnóstico de silla turca vacía cada vez más frecuente. Aunque clásicamente se ha considerado que la función hipofisaria casi siempre es normal, en algunos estudios publicados se ha demostrado que es una entidad en la que con frecuencia se encuentran alteraciones de la función hipofisaria que van desde hipopituitarismo total o parcial e hiperprolactinemia, recurriendo en ocasiones a tratamientos de suplementación hormonal y neuroquirúrgicos. En este artículo se revisa el concepto, clínica, fisiopatología y manejo de la silla turca vacía a la luz de la bibliografía actual.
