Differential diagnosis in neurological patients included syncope, epilepsy, metabolic disorders such as hypoglyce- mia, intoxication and ischemic stroke, the primary care physician should know about the pathophysiology of each of these conditions for a proper diagnosis that facilitates appropriate therapeutic approach, we report the case of a patient with a history of ischemic cerebrovascular events not well clarified, who presented clinical manifesta- tions of syncope. Artificial cardiac pacemaker was implanted with overall improvement of symptoms. The patient showed normal brain scans in both the current and previous neurological episodes.
