Abstract

Plasma cell leukemia is one of the rarest hematological malignancies worldwide. We present the case of a 45-year-old male patient who presented to the emergency department with chest pain as a clinical manifestation of leukostasis. Peripheral blood flow cytometry showed 37.80% pathological plasma cells, and a bone marrow biopsy documented infiltration of 50.68% pathological plasma cells. Therefore, plasma cell leukemia was diagnosed and a high-intensity chemotherapy regimen with the DCEP protocol was prescribed. This case is described is not only a rare hemato-oncological malignancy but also an uncommon cause of chest pain in adults. This case highlights the need for a thorough review, including the complete blood count, during the diagnostic approach to all chest pain cases in the emergency department.
